Assessment-only service terms and consent

Version: 2026-10-assessment-only-limits-v3

Purpose and scope

This is a limited physiotherapy assessment solely for a condition report, injury-related letter or employer documentation. It does not include treatment, a rehabilitation programme, prescribed exercises or ongoing monitoring, and does not enrol you in ongoing physiotherapy care. Any additional contact is to clarify or complete this assessment. Further treatment must be arranged separately. Automated tools prepare draft documents and analyze captured frames; the physiotherapist must personally review and approve the findings before signing. The practitioner may decline or limit documentation if the evidence is insufficient. A condition report is not a replacement for a doctor's note for time off. It may accompany your request for time off, exclusion or accommodation to an employer, school or service provider; the receiving organization decides whether to accept it and approve your request. Only employer forms specifically asking about leave or return to full duties may receive a recommendation of no more than seven days; ongoing limitations after that period require assessment by your family doctor. This practitioner is not your treating physiotherapist. This service is available once every two calendar months; earlier repeat requests require in-person physiotherapy. Professional assessment, safety, privacy and record-keeping obligations still apply.

This creates a professional relationship limited to this assessment and its documentation. It does not waive the practitioner's professional responsibilities.

How the assessment works

You complete questions and suitable recorded movement checks independently during your scheduled 10-minute intake window. A registered physiotherapist reviews the information afterward and may contact you by telephone or arrange a live video or in-person assessment if needed. The intake window is not 10 minutes of live one-to-one treatment, and a report is not guaranteed within 10 minutes.

Describe your current abilities; do not test maximum lifting or force painful movements. Camera estimates do not establish measured strength or lifting capacity. Stop if you feel unsafe. This service is not for emergencies; seek appropriate urgent care when needed.

Documentation and leave limits

Condition reports describe findings and functional limitations. A condition report is not a replacement for a doctor's note for time off. You may provide it with your request for time off, an exclusion or an accommodation to your employer, school, workplace or service provider. The receiving organization decides whether to accept it and approve your request. The report describes your functional findings rather than recommending a leave period. Only an uploaded employer form specifically asking about leave or return to full duties may receive a recommendation covering at most seven days. Ongoing limitations after that period require assessment by your family doctor. Standard employer letters do not recommend time off. The reviewing practitioner is not your treating physiotherapist.

This service is available once every two calendar months. Earlier repeat requests require an in-person physiotherapist.

Consent and your choices

You may ask questions before proceeding, refuse an assessment step or withdraw consent. Refusal may mean the practitioner cannot reach a supported conclusion or issue the requested documentation. Recording consent is requested separately before recording starts. An alternative is a live or in-person physiotherapy assessment arranged separately.

Records and authorized disclosure

The signed report or completed form is retained with relevant intake answers, recordings, consent, assessment findings, limitations, clinical reasoning and any further assessment contact. These together form the clinical assessment record; no separate SOAP format is required by this workflow. Automated tools prepare editable drafts from your answers and captured evidence; the practitioner must personally review and approve the conclusions. For employer requests, employer contact details are mandatory and employer paperwork is delivered directly to that employer. Requests without an employer PDF use a standard employer letter. Condition-report requests are delivered to the patient. The complete condition assessment record is retained for both services. You may request access to your clinical record through the clinic. Only the authorized documentation is sent to the recipient you confirm. Private clinical evidence is not routinely sent to your employer.

Records must be retained for at least 16 years from the last encounter or adulthood, whichever is later. Withdrawal does not automatically erase records subject to legal retention duties. Read the privacy information, including international storage and record-access rights.

Payment, coverage and eligibility

The client pays $30 CAD before the assessment starts. This is a one-time assessment booking, not a treatment subscription. We do not submit insurance claims or promise reimbursement. This independent intake and later review does not meet Pacific Blue Cross's published live-video physiotherapy benefit criteria. Confirm any coverage with your insurer.

This service currently supports clients aged 19 and over. Our physiotherapists are licensed in British Columbia. Eligibility depends on where you are physically located. WorkSafeBC/WCB forms are not accepted. Employer PDFs are screened before payment, including whether the required signatory and certification fit this independent physiotherapy assessment. Forms requiring a treating practitioner, physician or unsupported certification are rejected before Stripe opens.

These service-specific terms govern the assessment scope where general platform terms describe other PhysiPhone services.
